STEPS TO COMPLETING AN EVALUATION
- Complete all 3 assessments below (MAST, DAST, and SALCE).
- BOOK (Choose date and time).
- Make Credit Card payment.
- Receive first email (Booking Confirmed – Your receipt).
- Receive second email within 24 hours (Portal Sign-IN link).
- 3. Sign into Portal. Complete all forms/documents in portal and upload picture ID.
- Call office (832) 830-8662. Go over paperwork and receive virtual meeting link.
- Attend Virtual meeting (30 -45 mins) with Licensed Professional Counselor.
- Eavluation recommendation completed by the counselor and sent to C.S.O. within 48 hrs.
